This drug dosing tool computerizes the Queensland Health Safe and Quality Use of Medicine Unit’s 2012 Guidelines for Aminoglycoside Once Daily Dosing Guidelines for Adults.
Drugs included are gentamicin, tobramycin, and amikacin.
This dosing tool includes proper selection of weight to be used (IBW, ABW, or LBW) and integrates dosing frequency based upon creatinine clearance using the Cockcroft-Gault equation.
Dosing adjustments are also made based upon patient’s age.
