Checklist for meeting Ethical and Legal Obligations ( ChELO ) Is your patient unable to make decisions for themselves because of a critical illness or psychiatric condition? Appropriate treatment plans begin with an appropriate basis for consent. ChELO applies a legal consent pathway for incapable patients (Health Care Consent Act RSO 1996) in a checklist of clinically tested questions that prevent ethical or legal errors. The Aid to Capacity Evaluation is used for determination of decision making capacity. Free text notes can be typed during the interview. A communication bundle to support family understanding and values exploration is included. After completion of the 8 items a pdf can be exported for record keeping and measurement. A recommended format for the interview and other suggestions for successful communication are included. When taken as directed, a ChELO checklist meeting takes only 10-15 minutes and can serve as a foundation for an extensive values history or for discussion of the medically indicated treatment plan.
