Framework@PC provides healthcare professionals with evidence-based recommendations on diagnosis, assessment and management of patients with hypertension and diabetes in primary care settings in Hong Kong. It also provides alerts on important primary care related notices issued by the Department of Health of the Hong Kong Special Administrative Region via the feature “Notices to Doctors”.Framework@PC is developed by the Primary Care Office of the Department of Health of the Hong Kong Special Administrative Region. It is developed based on the “Hong Kong Reference Framework for Diabetes Care for Adults in Primary Care Settings” and “Hong Kong Reference Framework for Hypertension Care for Adults in Primary Care Settings” developed by the Task Force on Conceptual Model and Preventive Protocols under the Working Group on Primary Care.
