Requirements to Effectively Use The App:

A heart rate transmitter belt - either a low power bluetooth or an ANT+ (http://www.thisisant.com ) with a dongle (a small piece of hardware that attaches to the iPhone that when attached enables the iphone to receive signals in the format of ANT+).

You pick whatever Bluetooth (BLE) or ANT+ transmitter with dongle.

Please note that this app can continue to collect data for an active workout while in the background (using either Bluetooth or ANT+), which can dramatically decrease battery life.
