in some rare case your skin may not show up in the UCCW skin list since there's a new encryption feature introduced in Jelly Bean. Google Play uses feature while installing skins. 1) Install UCCW as already stated

2) add widget to the homescreen

3) select your desired widget

4) resize your widget if needed (depending on your launcher features)
