These lives may be gradually lost if the walls are hit when there is a syringe with poison. The good news is that the lost lives can be regained by touching the walls when the antidote appears.

The longer you manage to keep the cat alive the better score you get. The problem is that as time passes the difficulty increases as the box movements accelerate and it gets quite hard to control whether the kitty hits or not the box walls and when.
